Clinical Pharmacist
University Family Hlth Ctr SEPOSITION SUMMARY/RESPONSIBILITIES
Collaborates with pharmacy leadership, physicians and interdisciplinary team members in accordance with TJC, OSHA, Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations to deliver pharmaceutical care for eligible University Health System patients in the ambulatory setting. Conduct D.U.E. and patient/physician education activities as necessary. Functions as a source of drug information. Serves as a primary preceptor for pharmacy students and residents. Implements decisions set forth by the P&T Committee as written in the Hospital Formulary and comply with Health System policies and procedures.
EDUCATION/EXPERIENCE
A PharmD or Bachelor's of Science degree in Pharmacy is required. Completion of an American Society of Health System Pharmacy PGY 1 residency or 3 years of applicable experience is required. Completion of an American Society of Health System Pharmacy PGY2 residency in ambulatory care preferred. Must be licensed to practice pharmacy in the state of Texas. Certification by the Board of Pharmaceutical Specialties in Pharmacotherapy or Ambulatory Care preferred.
LICENSURE
Must be licensed to practice Pharmacy in the State of Texas. Must maintain an active preceptor license in the State of Texas.
