Master Social Worker
University Health System- San AntonioPOSITION SUMMARY/RESPONSIBILITIES
Manage and coordinate care among patients referred for mental or behavioral health services. Serve as primary link between the patient, provider, and Mental Wellness Institute. Responsibilities are comprehensive in scope with emphasis on patient plan and ensuring necessary and appropriate least restrictive treatment and support is provided, including brief intervention, referral follow-up and transition of care. Facilitates the delivery of services and support is provided efficiently and in a timely manner. Maintains good working, patient/provider/staff relationships. Provides skilled intervention for the support and/or resolution of patient and family crisis, problem-solving and decision support. Complies with and enforces University Health policies and procedures.
EDUCATION/EXPERIENCE
Master's degree in Social Work from an accredited institution of higher learning is required. Bilingual in Spanish/English is preferred.
LICENSURE
Current state licensure from the Texas Department of Human Services is required.
