The Oaks IIPOSITION SUMMARY/RESPONSIBILITIES
Work with the Director in the implementation of the Supporting Prevention in Neighborhoods (SPIN) Diabetes Program for University Health. This position will assist and guide patients through health prevention services to ensure that patients apply for and access health care services, participate in recommended care plans, and receive appropriate health information. This position will serve as the link between patients, the program partner, and healthcare access team in order to increase diabetes education and management. Additional activities will include but not be limited to patient tracking, data entry, and reporting.
EDUCATION/EXPERIENCE
Bachelor’s degree in public health, health education, social sciences, or similar field required. A minimum of 2 years or equivalent experience in the area of health education, public health, or case management. Experience proving educational presentations to both large and small groups as well as public speaking demonstrating effective communication skills is required. Previous experience navigating patients preferred. English/Spanish fluency is required.
