Robert B Green CampusPOSITION SUMMARY/RESPONSIBILITIES Assist Program Coordinator in the implementation of the CPRIT Get FIT Program for University Health. This position will assist and guide patients through health prevention services to ensure that patients are able to access and keep medical appointments, participate in recommended care plans, and receive appropriate health information. This position will serve as the link between patients, clinics and screening services in order to increase the utilization and compliance of colorectal screening and to provide education and support to the patients. Additional activities will include patient tracking, data entry, and reporting.
EDUCATION/EXPERIENCE
Bachelor’s degree in social sciences, health education or similar field or Community Health Worker certification is preferred. A minimum of 4 years or equivalent experience in the area of health education, public health, or case management. Experience proving educational presentations to both large and small groups as well as public speaking demonstrating effective communication skills is required. Previous experience navigating patients preferred. English/Spanish fluency is strongly desired.
