Responsibilities
The Cardiac Rehab Nurse coordinates the care of Phase II Cardiac Rehab patients in an outpatient program. The cardiac rehabilitation nurse is a registered nurse who possesses advanced knowledge and skill in the area of cardiac nursing.
Job Duties:
- Provides patient’s initial orientation and ongoing monthly assessments during rehab.
- Develops a rehabilitation plan that addresses the patient's risk stratification, risk factors, exercise and functional status.
- Develops patient rehabilitation discharge goals that promote transitioning the patient back into community health and exercise resources.
- Communicates rehabilitation plan and discharge by ordering physicians.
- Provides and documents patient care per department policy and external regulatory requirements.
- Performs patient education classes on risk factors of heart disease.
- May be required to make critical decisions concerning the initiative of emergency medical procedures and protocols.
- Performs and assists the Nurse Manager with other duties as they arise in the outpatient clinic setting.
Qualifications:
Minimum of three (3) years of experience as an RN with 1–2 years of experience working with cardiac patients - preferably on a telemetry unit or coronary care unit.
Excellent customer service skills
Requirements:
• Bachelor of Science in Nursing highly preferred
• Current TX RN License
• BLS and ACLS required
