Vice President, Care Transitions

POSITION SUMMARY/RESPONSIBILITIES

The Vice President, Care Transitions at University Health is charged with developing and maintaining a culture across the organization that is grounded in mutually beneficial partnerships among patients, families, departments and providers that fosters effective communication; dignity and respect for individual needs, values and cultural backgrounds; as well as collaboration and participation in the delivery of care and decision-making to improve patient outcomes and exceed the expectations of patients and families.  Research has shown that the more patients and their family members are involved with their own healthcare, the more improved are the health outcomes and the satisfaction level of the patient.

EDUCATION/EXPERIENCE

Bachelor’s Degree in Nursing and current registration with the Board of Nurse Examiners for the State of Texas is required.  Experience interpreting Customer Survey Response data.  Project management experience with demonstrable background in managing large, organization-wide projects. Key traits: strategic thinking, strong interpersonal skills, excellent presentation, communication and organizational skills and capabilities. Experience in a hotel, retail, healthcare or service industry and bilingual skills are preferred.

LICENSURE/CERTIFICATION

      Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required.
