Project Coordinator
The Oaks IIPOSITION SUMMARY/RESPONSIBILITIES
The Population Health area of CareLink is focused on implementing new programs aimed at improving members’ health outcomes and addressing barriers. This position will support these new programs by assisting with the implementation, working with high risk members, and reporting on results. Collaborates with community agencies and internal departments to design, implement and evaluate activities to assist CareLink members with resource identification and utilization, while ensuring initiatives employ health education methods and techniques that are developmentally appropriate and culturally sensitive in order to achieve program objectives. Will coordinate program activities with outreach staff including participating in health fairs, community events, conferences, wellness programs and other appropriate venues.
EDUCATION/EXPERIENCE
Bachelor’s degree in health education, social science, public health or equivalent experience is required. Experience in community health promotion, volunteer activity coordination, or health education in a community health setting is preferred. Experience with public speaking and/or teaching, individually or in groups, is required. English/Spanish fluency is preferred.
