POSITION SUMMARY/RESPONSIBILITIES

The Ambulatory Clinical Staff Pharmacist III oversees and directly contributes to the verification, preparation and dispensing of prescription orders for all ambulatory patients in accordance with all regulatory agencies, including but not limited to The Joint Commission (TJC), Occupational Safety and Health Administration (OSHA), Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations.  Activities include prescription order review and verification, counseling and educating patients on medication treatment and disease states, providing needed immunizations to members of the community, and is responsible for supervising the activity of pharmacy technician.  The ambulatory clinical staff pharmacist will continually strive to provide world-class service while assuring our patients receive their prescribed medications. The Ambulatory Clinical Pharmacist III demonstrates an expert level of competency related to ambulatory operations and will serve as a mentor and role model for Ambulatory Clinical Staff Pharmacists I and II in developing excellence in operational skills and for achieving departmental goals. The Ambulatory Clinical Pharmacist III will commonly be recognized by peers and other healthcare staff for their knowledge and expertise related to ambulatory operations, and will be recognized as a thought leader and innovator in this area of practice.

EDUCATION/EXPERIENCE

A Pharm.D. degree or a Bachelor of Science in Pharmacy from an ACPE-accredited School of Pharmacy is required. Completion of an ASHP-accredited Postgraduate Year Two (PGY2) Pharmacy Residency, a PGY1 plus 1 additional year of experience, or 4 years of experience in pharmacy is required.

LICENSURE/CERTIFICATION

Current Pharmacy and Pharmacy Preceptor licensure, or eligibility for licensure, in the state of Texas is required. Pharmacy-based immunization certificate, current American Heart Association Basic Life Support (BLS), Pediatric Advanced Life Support (PALS) and/or Advanced Cardiac Life Support (ACLS) certification as appropriate to area of practice, is required.  Certification by the Board of Pharmacy, advanced degree, or approved certification or certificate is required.
